This form is for administrators, ASB advisors, and/or teachers who work with student council. 

Leadership class is popular at my site. Most students select ASB as their elective, but not all students can be accommodated with their choice. What’s a fair way to select who gets to participate in Leadership? Use an application form system.

Having an application form helps make the decision fair and organized. 

Most application forms are not fully customizable unless you make create them yourself. But who has time for that? 

You’ll receive an ASB application, a job description page, and a teacher recommendation form.
