I love these worksheets for the first few days of school! It takes the pressure off of teacher and student as we lean into getting to know each other!
This was a great back-to-school activity my students did on the first day. I kept student copies to use for a daily "celebrity student" feature. My class gets 5 clues about a person and tries to guess who it is. It's a fun activity and students feel special.

Description

Looking for a way to get to know your students and build relationships in the classroom? Start the year off great with this set of student surveys for your middle and high school students! This is by far my favorite back to school activity. Your students will love filling it out and you get great insight into their lives. Includes 16 black and white surveys to print and 16 Google Slides with color to use digitally (for a total of 32 survey options). Within the 16 there are 8 with US spellings and 8 with UK spellings (think: color vs colour). The 8 surveys are similar with slightly different questions depending on age range. There are also 2 versions of the same survey, but with different seasons depending on if you would like to do the survey after summer or winter vacation. Survey questions focus on student interests, home life, strengths/weaknesses, goals, and hobbies.

Included at the bottom are directions for students to ask their teacher three "get to know you" questions. If you have a few extra minutes at the end of a class pull out the questions and answer for your students.

Let students personalize and get creative with their survey by coloring in the clip art or writing in each box with a different color. Show off student work - hang them up on a bulletin board for other students to see or for Back to School Night.

I like to save the surveys and return them to students on the last day of school. They get a kick out of seeing how much they've grown over the course of the school year and how their interests have changed.

No prep! Just print and go.

Digital version includes the same surveys, now with color! Formatted for Google Slides, each slide is flattened with text boxes for students to type in with their answers. Have students use different colors and fonts to customize for distance learning.
